Health Outcomes Advocate

We are seeking a dedicated professional to join our team as a Health Care Navigator in Wagga Wagga, Australia on a part-time basis. This role involves collaborating with care coordinators to address the diverse social needs that influence health outcomes.

The ideal candidate will take a holistic approach, helping individuals overcome barriers related to finances, transportation, housing, legal issues, and social isolation. Key skills and qualities include:
