
Healthcare Liaison Specialist

The primary objective of this position is to provide temporary support for Aboriginal and Torres Strait Islander Hospital Liaison Officers in various Metro South Health facilities. These officers collaborate with multidisciplinary teams to deliver culturally sensitive services that contribute to improved health outcomes and reduced health disparities among Aboriginal and Torres Strait Islander populations.
Hospital Liaison Officers offer assistance and support to Aboriginal and Torres Strait Islander patients and their families across Metro South Health facilities, encompassing patient visits, emergency services, hospitalization, and follow-up treatment.
This role involves working closely with healthcare professionals to ensure seamless delivery of care and services tailored to the unique needs of Aboriginal and Torres Strait Islander patients.
Key Responsibilities- Providing cultural support and liaison services to Aboriginal and Torres Strait Islander patients and their families.
- Collaborating with healthcare teams to develop and implement culturally sensitive care plans.
- Supporting patients during hospitalization, follow-up treatment, and transition to community-based care.
- Maintaining open communication channels with patients, families, and healthcare providers to ensure effective service delivery.
