HCP Case Manager PositionWe are seeking a skilled HCP Case Manager to join our team. As an HCP Case Manager, you will play a vital role in the delivery of high-quality patient care.
Main Responsibilities:- Collaborate with healthcare professionals to develop and implement comprehensive care plans that meet the unique needs of each patient.
- Conduct regular assessments and evaluations to ensure patients receive the best possible care.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Manage referrals, authorizations, and other administrative tasks to facilitate timely access to care.
- Analyze data and identify trends to improve care quality and outcomes.
- Minimum 1 year experience as a HCP case manager.
- PREFERRED Bachelor's degree in Nursing (However if you are an EN with good exposure in HCP funding we still encourage you to apply).
- Previous experience in care coordination or a related role in the healthcare industry.
- Strong organizational, communication, and interpersonal skills.
- Knowledge of home care regulations and industry best practices.
- Ability to handle and resolve conflicts effectively.
- Proficient in computer applications, including Microsoft Office and care management software.
- CPR and First Aid certification.
- PASSION for providing outstanding care to clients.
