With a strong commitment to compliance, compassion, and community, we are expanding our Home Care Package (HCP) program and seeking an experienced and dedicated Case Manager to join our team.



The Role

As a Case Manager, you will be responsible for the end-to-end management of Home Care Package clients — from intake and onboarding through to ongoing care plan reviews, compliance, and service coordination. You will work closely with our Nurse Coordinator, Operations, Rostering, and HR teams to ensure clients receive the highest standard of care.



Responsibilities
  • Manage a portfolio of HCP clients, ensuring care plans are developed, implemented, and reviewed regularly.
  • Conduct client intakes, including occasional travel for face-to-face onboarding and reviews.
  • Coordinate services in line with client goals, preferences, and budget.
  • Monitor and manage unspent funds, ensuring compliance with funding guidelines.
  • Maintain accurate records and documentation in line with regulatory requirements.
  • Liaise with multidisciplinary teams, families, and external providers to ensure seamless care delivery.
  • Contribute to the growth of the HCP program by supporting new client onboarding.


About You
  • Minimum 2 years' experience in Home Care Package case management (essential).
  • Strong knowledge of My Aged Care, HCP funding levels, and compliance requirements.
  • Excellent communication and interpersonal skills, with a client-focused approach.
  • Organised, proactive, and able to manage competing priorities.
  • Comfortable with occasional travel across Melbourne for client visits.
  • Current Australian Driver's Licence and access to a reliable vehicle.
  • Relevant qualifications in Aged Care, Community Services, Nursing, or related field.
