Position: Case Manager - Full Time
Location: Melbourne Northern Suburbs - South Morang
Responsibilities:
1. Participate in continuous quality improvement activities.
2. Conduct post-discharge follow-up to assess patient progress, address concerns, and prevent readmissions.
3. Generate reports on key performance indicators, client outcomes, and other relevant metrics.
4. Analyze data to identify trends, areas for improvement, and opportunities for optimization.
5. Manage the referral process, including receiving and processing referrals from healthcare professionals or agencies.
6. Ensure timely communication and coordination with referring entities.
7. Maintain accurate and up-to-date client records, including care plans, assessments, and communication logs.
8. Ensure compliance with privacy regulations and organizational policies.
9. Collaborate with physicians, nurses, therapists, and other healthcare professionals to optimize patient outcomes.
10. Participate in interdisciplinary team meetings to discuss and plan patient care.
11. Maintain regular communication with clients and their families to assess satisfaction and address concerns.
12. Communicate with care providers to ensure they are well-informed about the clients' needs and preferences.
13. Collaborate with healthcare professionals, such as nurses and therapists, to coordinate care plans.

Qualifications
1. 1 + year experience as a HCP case manager.
2. Preferred Bachelor's degree in Nursing (However if you are an EN with good exposure in HCP funding we still encourage you to apply).
3. Previous experience in care coordination or a related role in the healthcare industry.
4. Strong organizational, communication, and interpersonal skills.
5. Knowledge of home care regulations and industry best practices.
6. Ability to handle and resolve conflicts effectively.
7. Proficient in computer applications, including Microsoft Office and care management software.
8. CPR and First Aid certification.
9. Passion for providing outstanding care to clients.
Preferred Skills
1. Clinical Expertise: Proven experience in providing direct client care and ensuring safety.
2. Ability: Ability to develop/implement and evaluate organizational and client-focused documentation in the system.
3. Government Funding HCP /CHSP: Good understanding of the government funding environment for aged care (All levels of Home Care Packages).
4. Hospital Discharge: Good understanding of the hospital discharge environment.
5. Follow Up/Liaison: After any interactions with hospitals or health professionals, follow up as needed. This may involve scheduling follow-up appointments, obtaining test results, or clarifying instructions. Having a good understanding of how to liaise with hospitals and health professionals is a must.
6. Health Assessments: Good understanding of health assessments and developing care plans to meet clients' care needs.
7. Multitasking and Time Management: Ability to multitask and manage time effectively.
8. Communication: Excellent verbal and written communication skills.
9. Computer Skills: Intermediate to advanced computer skills, including familiarity with CRM systems.
