About the Role:
We are seeking a skilled Grade 2 Social Worker to join our Paediatric Endocrinology and Diabetes team at Monash Children's Hospital. As a key member of our multidisciplinary team, you will provide high-quality social work services to patients and their families, supporting them through complex medical conditions.
Key Responsibilities:
- Provide psychosocial assessments, problem-solving, and discharge planning services to patients and families
- Support families in coping with acute and chronic illnesses, including end-of-life issues
- Collaborate with healthcare professionals to develop and implement holistic care plans
- Participate in quality improvement initiatives and contribute to a culture of excellence
About You:
To succeed in this role, you will require:
Minimum 2 years of clinical social work experienceBachelor of Social Work or Master of Social Work Qualifying degreeEligibility for ordinary membership of the Australian Association of Social WorkersExperience in acute healthcare settings, child protection, or working with children and familiesWhat We Offer:
