Clinical Social Worker

Monash Health is seeking an experienced Grade 2 Social Worker to join the Clayton Adult Mental Health Program. The successful candidate will provide specialist social work interventions and short-term care coordination support to individuals in the Clayton bed-based mental health service.
Key Responsibilities- Conduct screening, assessment, and access to community supports to reduce psychosocial stressors impacting mental health.
- Link with clinics in the Clayton catchment to achieve seamless transition for clients and involve families and carers in recovery.
- Work within a multidisciplinary team to provide support to consumers receiving direct care and ongoing treatment.
- Qualified and experienced Social Worker with postgraduate experience providing client-centred care.
- Ability to adapt to changing settings with a hardworking and enthusiastic nature.
- Dynamic Allied Health professional passionate about making a positive contribution to patient health.
- Excellent communication and interpersonal skills, with a dedication to safe best practice and quality care.
- Unwavering commitment to delivering patient-centred care, with a positive attitude, compassion, and ability to work collaboratively in a team.
- AASW accredited Bachelor degree or entry-level Master's Degree of Social Work.
- Good organisational skills and a positive attitude towards work.
- Experience in working with culturally and linguistically diverse communities.
- Able to work autonomously, with a friendly, outgoing, and reliable demeanour.
- Well-developed clinical assessment and complex discharge planning skills.
