About the Role:
We are seeking a highly skilled and experienced Grade 2 Social Worker to join our team in the Department of Paediatric Endocrinology and Diabetes at Monash Children's Hospital.
Key Responsibilities:
- Provide psychosocial assessment, problem-solving, and discharge planning services to patients and their families.
- Support families in coping with acute and chronic illness, and provide grief and bereavement support as needed.
- Work collaboratively with the multidisciplinary team to provide holistic care and support to patients and their families.
- Participate in quality improvement and research projects to enhance patient care and outcomes.
Requirements:
- Minimum 2 years of clinical social work experience in a healthcare setting.
- Bachelor of Social Work or Master of Social Work Qualifying degree, or equivalent as assessed and approved by the Australian Association of Social Workers (AASW).
- Eligible for ordinary membership of AASW.
- Experience in an acute healthcare setting, working with children and families, and/or with Child Protection.
