About the Role
This exciting opportunity is available in the Clayton Adult Mental Health Program at Monash Health. As a successful applicant, you will work closely with the multidisciplinary team to provide high-quality care to consumers. Your key responsibilities will include:
- Screening, assessment, and access to community supports to reduce psychosocial stressors impacting mental health
- Linking with clinics in the Clayton catchment to ensure seamless transitions for clients and their families
- Providing specialist social work interventions and short-term care coordination support

Requirements
To be considered for this role, you will need:
- An AASW accredited Bachelor degree or entry-level Master's Degree of Social Work
- Good organisational skills and a positive, enthusiastic attitude towards work
- Experience in working with culturally and linguistically diverse communities
- Able to work autonomously
- A friendly, outgoing, and reliable demeanour
- Well-developed clinical assessment and complex discharge planning skills
- A current Victorian driver's license
