About the Role

We are seeking a highly skilled and dedicated Non-Government Service Coordinator to join our team at SA Health. As a key member of our Northern Community Mental Health team, you will play a vital role in developing, implementing, and evaluating quality services for consumers in the Northern Adelaide Local Health Network.

Key Responsibilities
  • Facilitate the identification and referral of consumers in acute/sub-acute settings into appropriate non-government psychosocial programs.
  • Identify and refer consumers into other programs where necessary.
  • Work with staff to ensure consumers are identified, allocated, provided with care coordination, referred to the appropriate non-government organisation, reviewed, exited or transferred as appropriate to longer term support services.
  • Optimise patient functional and health outcomes in this specialty area.
  • Deliver quality services and provide clinical leadership across Mental Health Services, providing support and supervision to less experienced staff.
Requirements
  • Hold an appropriate degree or equivalent qualification for one of the following Allied Health disciplines working in the service:
  • Occupational Therapist with Occupational Therapy Australia
  • Master of Clinical Psychology or equivalent qualification recognised by the PsyBA and general registration with PsyBA and an endorsed area of practice in Clinical area of practice
  • Appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers
