Family Support Role Overview

As a dedicated professional, you will play a pivotal role in supporting families within the Integrated Family Support Service to enhance their safety, stability, and overall wellbeing. You will engage with families to develop effective plans for long-term positive outcomes, actively work with them to identify goals that promote safety, well-being, and belonging for their children, and undertake case management tasks including assessments, reviews, reporting, referrals, and administrative functions.

Key Responsibilities:
  • Engage with families to develop effective strategies for long-term positive outcomes.
  • Actively collaborate with families to identify goals that promote safety, well-being, and belonging for their children.
  • Engage families in their homes and other suitable locations to overcome barriers to engagement.
  • Undertake case management tasks including assessments, reviews, reporting, referrals, and administrative functions.
  • Develop and update individualised case management plans for all clients.
  • Coordinate and participate in stakeholder meetings with other services and clients.
Requirements for Success

To be successful in this role, you will require:

  • A degree or diploma in Community Services, Human Services, Social Work, Social Sciences, Psychology, or another relevant field.
  • Experience in case management of families facing multiple and complex challenges such as mental health, disability, and domestic and family violence is highly desirable.
  • Strong administration skills, including reporting, case notes, and general documentation.
  • Exceptional communication, negotiation, and influencing ability, able to relate to children, parents, and other service providers.
  • The ability to form partnerships and connections with other professionals and organisations.
  • Demonstrated capacity to manage time effectively while managing a caseload of complex clients, maintaining professional boundaries and confidentiality.
  • Confident working autonomously and in one-to-one sessions with clients.
  • A sound level of computer literacy, including proficiency in Microsoft Office packages and client case management systems.
  • A full driver's license with no restrictions.
