We are seeking a skilled and compassionate Care Coordinator to join our team. As a key member of our organization, you will play a vital role in planning and managing care services for clients receiving in-home support.
- Conduct initial assessments to develop person-centred care plans that cater to individual needs.
- Coordinate and schedule care services with accuracy and efficiency, ensuring seamless delivery of support.
- Support carers with guidance, resources, and regular check-ins to enhance their effectiveness.
- Maintain compliance with care standards and documentation to guarantee quality care.
- Ensure clients feel heard, respected, and supported at all times, fostering a positive and empowering environment.
To succeed in this role, you will need:
- Experience in care coordination, case management, or a similar field.
- A background in aged care, disability support, or community services.
- Excellent communication, time management, and problem-solving skills.
- A strong ethical approach and attention to detail.
- Confidence using technology and care management systems.
