Job Title: Magistrates Early Referral Case Manager

About this Role:

Our organization is seeking a highly skilled Magistrates Early Referral into Treatment (MERIT) Case Manager to join our team. As a MERIT Case Manager, you will work with individuals who have drug and alcohol issues, providing assessment, case management, support, counseling, and treatment.

MERIT is a court diversion program that offers a unique opportunity for defendants to address their substance use, associated lifestyle, and health issues. You will engage with people before the court with a history of or current drug use, providing multi-disciplinary team assessment and case management to improve their lifestyle and health, reducing re-offending.

Key Responsibilities:

  • Provide assessment, intensive case management, support, counseling, and treatment to defendants with drug and/or alcohol issues in Local MERIT Courts and MERIT treatment areas.
  • Collaborate with a multi-disciplinary team, including government and non-government services, and community groups, to provide holistic treatment and prevention approaches.
  • Develop and implement treatment plans, referrals, group work, and community follow-up, ensuring continuity of care for individuals and their families.
  • Liaise with local courts, police, and other stakeholders to facilitate smooth program delivery.
  • Maintain accurate records, participate in regular case conferences, and contribute to program evaluation and development.
  • Demonstrate leadership within a diverse range of inpatient and/or outreach settings, working effectively within a multidisciplinary team or autonomously.
  • Possess high-level communication skills, advanced written, oral communication, and competent computer skills.
  • Hold a current unrestricted driver's license and be willing to drive/travel as required, potentially involving long distances and overnight stays.
  • Comply with NSW Health policy directives, including Occupational Assessment, Screening, and Vaccination against Specific Diseases requirements.
  • Respect Australian Work Rights, accommodating reasonable adjustments throughout the recruitment process if necessary.

Requirements:

  • Tertiary qualifications and current registration with the Australian Health Practitioners Regulation Agency (AHPRA) as a Registered Nurse, Occupational Therapist, or Psychologist.
  • Tertiary qualifications for Social Worker, Counsellor, or Welfare Officer with eligibility for membership in relevant professional associations.
  • Demonstrated knowledge and experience in holistic treatment and prevention approaches, applicable to people with complex needs.
