About the Role:
The Service Delivery Coordinator is a vital position in our organization, responsible for ensuring the delivery of high-quality support services to clients. The successful candidate will play a crucial role in coordinating support workers, scheduling client appointments, and maintaining accurate records.
Key Responsibilities:
- Schedule support workers to optimize client interactions and minimize travel time.
- Enter and maintain client information accurately in our database, including contact details, service agreements, and alerts.
- Communicate schedules efficiently, record changes, and maintain records of communications with clients and staff.
- Monitor resource availability and ensure effective utilization.
- Collaborate with stakeholders to deliver exceptional support services.
- Regularly review scheduling systems to improve efficiency and effectiveness.
Documentation:
- Maintain up-to-date documentation of client information, including service agreements, contact details, and client profiles.
- Assist in updating client documentation, including service agreements and other relevant paperwork.
Quality and Continuous Improvement:
- Stay informed about industry developments, including aged care programs and regulatory requirements.
- Provide feedback on support worker supervision needs and escalate concerns in a timely manner.
- Report incidents, complaints, hazards, and near misses related to service delivery.
Training and Professional Development:
- Pursue ongoing professional development to reflect changing industry needs and requirements.
- Attend mandatory training sessions and meetings.
Requirements:
- Certificate III Individual Support or equivalent experience
- Excellent communication and interpersonal skills
- Able to work independently and as part of a team
- Proficient in MS Office and other relevant software
