Progressive Neurological Disease Service Liaison

The Progressive Neurological Disease Service (PNDS) at Calvary Health Care Bethlehem is a centre-based outpatient clinic, offering multidisciplinary assessment, education and management services for people with diagnosed progressive neurological diseases. The service aims to enhance the quality of life of patients and families affected by these conditions through planning, coordinating and delivering high-quality multidisciplinary care to meet their physical and psychosocial health and disability support needs.

Job Description

As a Clinical Liaison within our team, you will work closely with an allocated caseload and medical team member to provide comprehensive patient assessments, including detection and recognition of symptoms related to treatment, disease process or disease progression. You will play a central role in bridging the divide between services within our hospital and external health, community care, disability and other services, through active care coordination.

Key Responsibilities
  • Use 'disease specific' knowledge to provide comprehensive patient assessment
  • Plan and provide person-centred care for people with progressive neurological disease
  • Contribute to ongoing service design, evaluation and improvement to ensure best outcomes for patients
Requirements

To be successful in this role, you will have:

  • Experience in one or more of: Progressive Neurological Diseases, Palliative Care, Mental Health, Disability or Community Health
  • Interest in developing disease-specific knowledge, including a knowledge of community resources and pathways
  • Ability to work independently and autonomously in complex situations
  • Well-developed communication and computer skills
