About the Role

The Client Liaison Officer will play a key role in our team, undertaking assessment, support planning, packaged care coordination, and case management activities designed in collaboration with the client to assist them in achieving their identified goals.

This role will support clients to manage and monitor their budget to ensure care packages are financially sustainable and monitor the services provided to ensure they are following relevant service models and guidelines.

Key Responsibilities

  • Undertake assessments and develop support plans in collaboration with clients
  • Coordinate packaged care and case management activities
  • Support clients with budget management and financial sustainability
  • Monitor services provided to ensure they are following relevant service models and guidelines
  • Provide exceptional communication and advocacy skills to support clients

Requirements

  • Experience in aged care programs and assessments
  • Proven understanding of packaged care and case management
  • Ability to engage clients and work effectively with them to achieve identified goals
  • Exceptional communication and advocacy skills
  • Certificate IV in Health, Community Services, Aged Care, or Disability Support
  • Current Driver Licence and National Police Certificate
