About the Role

The Client Liaison Officer/Team Leader will play a key role in our team, working closely with clients to assess their needs and develop personalized care plans. This role requires strong communication and interpersonal skills, as well as the ability to work effectively with clients, families, and other stakeholders.

Key Responsibilities

  • Conduct assessments and develop care plans in collaboration with clients and their families
  • Coordinate and manage care packages to ensure clients receive the support they need
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed
  • Provide support and guidance to clients with budget management and financial sustainability
  • Work collaboratively with other team members to achieve shared goals and objectives

Requirements

  • Experience working in aged care or community services
  • Proven understanding of assessments, care planning, and case management
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a team environment
  • Certificate IV in Health, Community Services, Aged Care, or Disability Support
  • Current Driver's License and National Police Certificate
