About the Role

The Client Liaison Officer will undertake assessment, support planning, packaged care coordination, and case management activities in collaboration with the client to achieve their identified goals. This role supports clients to manage and monitor their budget to ensure care packages are financially sustainable and monitors the services provided to ensure they are following relevant service models and guidelines.

Key Responsibilities

  • Experience with Aged Care programs is highly regarded
  • Proven understanding of assessments, packaged care, and case management
  • Ability to engage clients and work effectively with them to achieve identified goals
  • Support and guidance to clients with budget management and financial sustainability
  • Experience in monitoring and assessing services provided to ensure they are following service models and guidelines
  • Exceptional communication skills – both written and verbal – and the ability to relate to a wide range of people
  • Ability to advocate strongly for your clients and the services they require
  • A sound understanding of the community care system and the people who utilise this service
  • Diplomacy and negotiation skills; people management capabilities

Requirements

  • Certificate IV in Health, Community Services, Aged Care, or Disability Support
  • Current Driver Licence
  • National Police Certificate
  • Qualifications relating to Case Management are highly valued
