Culturally Responsive Support Worker

We are seeking an experienced and compassionate individual to join our team as a Support Worker. The successful candidate will provide outreach support to patients in a culturally sensitive manner, linking them to relevant services and supporting them through the NDIS process.
Key Responsibilities- Deliver high-quality patient support services that meet the needs of vulnerable populations.
- Link patients to their care providers and facilitate access to the NDIS.
- Support patients to navigate the NDIS process, including attending appointments and completing necessary documentation.
- Maintain accurate records on the patient information system and provide regular reporting.
- Pursue ongoing professional development opportunities relevant to the role.
- Minimum Certificate IV qualification in a relevant field or extensive experience in the disability/NDIS sector.
- Excellent communication and interpersonal skills with the ability to work effectively with diverse populations.
- In-depth knowledge and understanding of the NDIS and its operation.
- A high level of confidentiality and tact, compassion, and empathy.
- Well-developed computer skills with the ability to maintain accurate records.
- Aboriginal and/or Torres Strait Islander heritage.
