Patient Care Coordinator Opportunity

We are seeking a Patient Care Coordinator to join our team. As an integral part of the Admissions Team, you will manage the discharge planning function in conjunction with Nurse Unit Managers and other stakeholders to optimize patient service and financial objectives.
The successful candidate will work closely with Admissions Coordinators to ensure even distribution of admissions and discharges, actively communicate with other service providers regarding ‘at risk’ patients who may require coordination of multiple services, collaborate with medical and multi-disciplinary staff, internal and external to the organization, coordinate timely follow up of patients after their inpatient admission and respond appropriately to any clinical concerns that may be raised.
Requirements- Possession of qualification in Nursing with at least 3 years post registration clinical experience in mental health
- Registration with the Nursing and Midwifery Board of Australia as a Registered Nurse
- High-level communications and interpersonal skills, with a demonstrated ability to understand and respond to stakeholder needs
- Exceptional organisational and time management skills with an ability to multi-task
- Well-developed analytical and problem solving skills
- Demonstrated ability to work collaboratively in a multidisciplinary healthcare environment
