Support Worker

Action on Disability within Ethnic Communities Inc. is seeking a skilled and compassionate Support Worker to join our team in providing high-quality care and support to individuals in need.
Key Responsibilities- Deliver personalized care and support services to clients in their homes and communities, as per their individualized care plans.
- Assist clients with daily living activities, such as personal care, domestic assistance, and community access support.
- Develop and maintain strong relationships with clients, their families, and other stakeholders to ensure comprehensive care and support.
- Collaborate with multidisciplinary teams to coordinate care and services, and to ensure seamless transitions between care settings.
- Document and report client progress, concerns, and changes in their condition, as required.
- Participate in ongoing professional development and training to enhance skills and knowledge in care and support.
- Certificate III in HACC or equivalent qualification in aged care or disability support.
- Certificate III in Individual Support or equivalent qualification.
- Current Level 2 First Aid and CPR certification.
- Previous experience in disability or aged care, preferably in a similar role.
- Valid driver's license and reliable transportation.
- Ability to work independently and as part of a team, with minimal supervision.
- Formal qualifications in social work, community development, or related fields.
- Excellent communication and interpersonal skills.
- Ability to work with diverse client populations, including those from culturally and linguistically diverse backgrounds.
