Australian Home Care Team Lead

Salary

$80,000 - $110,000 per annum, depending on experience.

Job Responsibilities

The Australian Home Care Team Lead will be responsible for overseeing a team of telehealth Home Care Package Care Managers or Relationship Managers. Key responsibilities include:

  • Ensuring the delivery of exceptional customer service to clients;
  • Supporting clients to live safely, well and independently at home while complying with Aged Care Quality Standards;
  • Oversight of clients at risk;
  • Guidance on high-quality goal-oriented care planning;
  • Ensuring all packages are financially viable;
Required Skills and Qualifications

To be successful in this role, you will require:

  • Excellent leadership skills and a passion for guiding and mentoring;
  • Problem-solving abilities with the ability to think critically;
  • Strong interpersonal and verbal/written communication skills;
  • Ability to work at a fast pace with effective time management skills;
  • Ability to develop relationships with clients, their family and employees;
  • Willingness to learn new systems, policies and procedures;
  • Experience in Aged Care, Community Home Care and existing knowledge of Home Care Packages (HCP) and Commonwealth Home Support Program (CHSP);
  • Intermediate MS Office computer skills;
