Job Overview

We are seeking an experienced Assistant Care Manager to join our team on a 3-month fixed-term contract basis. As an Assistant Care Manager, you will play a critical role in delivering person-centered care, managing client service delivery, and ensuring compliance with relevant legislative and industry standards.

Responsibilities

  • Care Planning: Develop, monitor, and reassess care plans in consultation with clients, their families, and other healthcare professionals.
  • Client Service Delivery: Manage client service delivery across all levels under the government-funded CHSP and Home Care Packages programs.
  • Collaboration and Communication: Work closely with internal teams, including Care Managers, Nurses, Schedulers, Administrators, Regional, and General Managers, to facilitate seamless service delivery.
  • Compliance and Quality Assurance: Ensure compliance with all relevant legislative and industry standards, maintaining high standards of care and quality assurance.
  • Financial Management: Monitor the budget of care packages to maximize funding for clients and ensure clients understand service fees.

Requirements

  • Certification: Certificate III in Individual Support, Aged Care, Disability, or similar qualifications.
  • Experience: Experience in aged care, with knowledge of Home Care Packages, Commonwealth Home Support Program, and Aged Care Quality Standards is desired.
  • Certifications: Current First Aid and CPR certificates.
  • Licenses: The right to work in Australia and a current Australian driver's license, vehicle registration, and access to your own car with comprehensive car insurance.
  • National Police Clearance: National Police Clearance or willingness to obtain (myHomecare will organize via WorkPro at no cost).

Salary and Benefits
