Job Description:

The Client Outcomes Lead is responsible for overseeing and operationalising the client services team, leading day-to-day operations and ensuring smooth service delivery. They manage a quarter of a case load, including leave management of team members and other caseloads when required.

They guide participants from initial setup, facilitate support worker matching and ongoing management for participant relationships with Hearth Support Services. The Client Outcomes Lead is accountable for leading and supporting the Support Worker team to deliver quality, person-centred and client-focused outcomes.

Mentoring, coaching and developing the broader Relationship Manager team are key responsibilities. Additionally, they contribute to the strategic direction for the Relationship Management operational function by establishing best practice frameworks.

Responsibilities include:

  • ">
  • Contribute to the development of the strategic direction for the Relationship Management operational function">
  • Foster and develop positive and supportive relationships with participant families to fully understand their needs">
  • Coordinate timely responses to ensure participant requests and feedback are managed appropriately">
  • Develop and maintain positive relationships with key external disability service providers and stakeholders">
  • Oversee and ensure support worker wellbeing, training, performance development and opportunities for career growth">
  • Ensure high-quality service and support is delivered to participants that complies with Hearth's Participant Service and NDIS quality standards and legislative requirements">
  • Determine participant requirements through initial meetings, guided development of care plans, facilitate care team meetings, conduct quarterly reviews and contribute to NDIS plan reviews">
  • Caseload management of direct reports and team, including intake management">
  • Lead, coach, develop and support teams to provide responsive, person-centred support that enables participants to progress toward their individualised NDIS plan goals">
  • Work closely with the Intake Team to assess and progress new referrals">
  • Ensure participant requirements are met throughout the care process">
  • Maintain accurate documentation, including Service Agreements, Management Plans, Behaviour & Medication plans and High Intensity Supports documentation">
  • Document case notes and client reviews that reflect goal planning in line with participant NDIS plans">

Requirements:

">

A minimum of 15 years leadership/management experience in the disability sector, including developing strong relationships and managing service delivery teams. Qualifications in Disabilities, Community Services or related disciplines are also required.

Extensive experience in person-centred disability support services and plan principles is necessary. Strong understanding of disability sector regulatory, legislative, quality and compliance standards and policy and procedures are also essential.

High-level decision making and problem-solving skills are required, as well as excellent interpersonal and communication skills.
