This leadership position requires advanced clinical expertise, critical thinking, and project management skills to design and implement tailored care plans that empower patients to manage mental health, chronic pain, and substance use challenges outside of emergency settings.
- Design personalised care strategies that support patients in managing their health effectively.
- Lead the development and evaluation of our Complex Care and Frequent User Management Program, ensuring it remains responsive to evolving patient and system needs.
- Collaborate with clinical teams and senior leaders to drive strategic initiatives that promote better health outcomes and deliver impactful service improvements.
- Combine advanced clinical practice with project leadership to develop innovative solutions that make a lasting difference in patient care.
- Current registration as a Registered Nurse with Authority to Practice from the Australian Health Practitioners Regulation Agency (AHPRA).
- Minimum seven years' full-time post-graduate experience, including five years' equivalent experience in the specialty field.
- Postgraduate nursing qualifications relevant to the specialty field or evidence of working towards such qualifications.
- High level of clinical expertise, critical thinking, and advanced clinical reasoning in delivering person-centred, evidence-based, and trauma-informed care and case management.
