
Care Coordinator

Job Summary:
We are currently seeking dedicated mental health professionals who can provide high-quality, client-centered care in an integrated stepped care model.
The ideal candidate will possess strong communication and interpersonal skills, enabling them to work effectively within a fast-paced environment. They should be committed to delivering exceptional patient outcomes while maintaining a collaborative team approach.
- Key Responsibilities:
- Deliver comprehensive mental health assessments, treatments, and/or case coordination services
- Collaborate with multidisciplinary teams to provide seamless care delivery
- Develop and maintain effective relationships with clients, families, and community partners
- Engage in ongoing professional development to enhance skills and knowledge
Requirements:
- Qualifications: Registered Psychologist, Mental Health Accredited Nurse, Mental Health Accredited Social Worker, or Mental Health Occupational Therapist
- Relevant experience working in mental health settings, preferably in outreach capacities
- Strong communication and interpersonal skills
- Ability to work autonomously and as part of a collaborative team
