As a Client Liaison Officer at the Atlam Group, you will be responsible for undertaking assessments, support planning, and case management activities designed in collaboration with the client to assist them in achieving their identified goals.
Key Responsibilities:
- Support the client to manage and monitor their budget to ensure care packages are financially sustainable.
- Monitor the services provided to ensure they are following relevant service models and guidelines.
- Collaborate with clients to develop and implement personalized care plans.
- Provide ongoing support and guidance to clients to ensure they are achieving their goals.
Requirements:
- Certificate IV in Ageing, Lifestyle & Health, or Disability, or equivalent.
- Current National Police Certificate, or willingness to obtain.
- Current Driver's License and own reliable vehicle.
Desirable Skills:
- Proven understanding of assessments, packaged care, and case management.
- Ability to engage clients and work effectively with them.
