About the Role

VMCH is seeking an experienced Care Coordinator Specialist to join our team. As a Care Coordinator Specialist, you will play a vital role in delivering high-quality care and support to consumers with diverse and complex needs.

Your Responsibilities

  • Develop tailored care plans that cater to the unique needs of each consumer.
  • Manage consumer budgets effectively and ensure compliance with Aged Care Reforms and Quality Standards.
  • Collaborate with various stakeholders, including healthcare professionals and community service providers.
  • Communicate effectively with consumers, their families, and carers to ensure a smooth delivery of care services.
  • Travel to regional Victoria as required to provide care and support services.

About You

To be successful in this role, you will need:

  • A health-related tertiary qualification or equivalent experience in a related field.
  • Previous experience as a care manager developing care plans and managing consumer budgets.
  • Excellent communication and interpersonal skills.
  • A driver's license and willingness to travel.
  • A police check.

Salary

We offer a competitive salary package, estimated at $80,000 - $110,000 per annum, depending on your level of experience and qualifications.
