Care Coordinator Specialist

SalaryThe salary for this role is estimated to be around $65,000 - $85,000 per annum, depending on experience and qualifications.
Job DescriptionKey Responsibilities:
- To develop tailored care plans that meet the individual needs of consumers;
- To manage consumer budgets effectively and ensure compliance with Aged Care Reforms and Quality Standards;
- To collaborate with stakeholders and understand referral pathways;
- To uphold the highest standards of care and service delivery.
About You:
- You will have a health-related tertiary qualification or in a related field;
- You should be prepared to adapt to changing caseloads and demonstrate a flexible mindset;
- You will have previous experience as a care manager developing care plans and managing consumer budgets;
- You will have a strong understanding of Aged Care Reforms and Quality Standards;
- You must be willing to travel to regional Victoria when required;
- You will have aged care knowledge and customer service skills;
- You will possess a driver's license and be able to pass a police check.
