Job Description

This is a unique opportunity to make a real difference in the lives of older adults—helping them stay well, safe, and connected.

As a nurse with Catholic Healthcare, you will draw on primary health care principles to deliver holistic, person-centred care to clients experiencing mental health challenges, chronic conditions, and complex needs.

You will provide direct clinical nursing care to our clients in their homes, in line with best practice nursing procedures. You will also develop a holistic nursing care plan in collaboration with the client, carer, Care Advisor, and Allied health where relevant.

The Day-to-Day This Role Looks Like:

  • Providing direct clinical nursing care to our clients in their homes, in line with best practice nursing procedures
  • Conducting clinical assessments, dressings, medication assessment, and catheter changing
  • Developing a holistic nursing care plan in collaboration with the client, carer, Care Advisor, and Allied health where relevant
  • Providing safe, quality nursing practice to clients with specific needs and/or complex needs such as wound care, palliative care, education, and support
  • Liaising with all parties involved in the care of the client to maintain holistic healthcare and wellbeing
  • Ensuring accurate and timely completion of documentation

Key Responsibilities

To succeed in this role, you'll need:

  • A genuine passion for providing high-quality care to the elderly
  • Current AHPRA registration following completion of a Bachelor of Nursing or similar qualification
  • Minimum 3 years post-graduate nursing experience
  • Experience in clinical assessments
  • Initiative and ability to work independently
  • Strong computer skills, especially Microsoft Office products, and ability to learn new systems quickly
  • NSW driver's license and reliable vehicle
