We are seeking a highly motivated and compassionate Caregiver Specialist to join our team in Celina, OH, Coldwater, OH, or St. Marys, OH. As a key member of our Supported Living team, you will provide personalized support and care to individuals with disabilities, enabling them to live independently and reach their full potential.
Key Responsibilities:
1. Provide daily living assistance, including personal care, meal preparation, and household management.
2. Develop and implement personalized support plans to meet the unique needs of each individual.
3. Collaborate with our interdisciplinary team to ensure seamless care coordination and communication.
4. Maintain accurate records and reports, adhering to regulatory requirements and company standards.
Requirements:
1. High school diploma or equivalent required; degree in a related field preferred.
2. 1+ year of experience in a care-related field, preferably in Supported Living or a similar setting.
3. Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
4. Ability to lift up to 50 pounds and perform physical tasks as needed.
