Job Title: Care Manager Overview We are seeking a skilled and compassionate professional to deliver high-quality care to residents while promoting independence and lifestyle choices that suit their needs. As a key member of our staff, you will be responsible for developing, monitoring, and implementing personalized care plans. Key Responsibilities:
  • Ensure the delivery of high-quality and personalized care to all residents
  • Develop, monitor and implement nurse care plans, assessing, planning and evaluating existing care plans
  • Perform comprehensive assessments and investigate issues and incidents
  • Participate in and ensure all compliance is adhered to in accordance with aged care standards and policies and procedures
  • Actively supervise, train and mentor RNs and PCWs
Required Skills and Qualifications
  • Ability to work full time, Monday-Friday
  • Experience as an Aged Care Manager/Coordinator
  • Demonstrated working knowledge of current aged care standards, accreditation and auditing processes, and AN-ACC
  • Personal and professional commitment to delivering high-quality care, developing yourself and your staff, and continuous improvement
  • Solid leadership skills along with excellent communication and interpersonal skills
  • Current flu and COVID Vaccinations
