Caregiver - Hospice Admissions Nurse

As a Hospice Admissions Nurse, you will be responsible for completing comprehensive assessments of patients and families to determine their medical, emotional, and social needs. You will develop individualized plans of care that address the unique needs and goals of each patient and family.
- You will utilize health assessment data to identify nursing diagnoses and develop interventions to improve patient outcomes.
- Administer medications and treatments as prescribed by physicians and participate in interdisciplinary team meetings to ensure comprehensive care.
- Counsel patients and families on self-care techniques and provide education on managing chronic conditions.
The responsibilities of this role include travel throughout the territory to complete admissions required. The schedule for this full-time position is Monday-Friday 11:00AM-7:00PM.
Requirements:
- A graduate of an accredited nursing school with current licensure to practice in the state of operation is required.
- At least one year of nursing experience is necessary, three or more years preferred.
- Experience in hospice or a similar setting is preferred.
Key Qualifications:
- Demonstrated knowledge of the hospice philosophy of care and commitment to providing exceptional patient care.
