Career Transition Specialist

Pinnacle Rehab is an award-winning workplace rehab provider with a goal of achieving quality recovery and return-to-work outcomes for individuals who have been injured at work.
- Support individuals to regain confidence and independence through a collaborative team culture, extensive induction, and onboarding into the industry of occupational rehabilitation.
Our dedicated consultants work closely with clients to identify barriers preventing them from returning to work and develop skills needed to conduct motivational interviews as well as influencing stakeholders to achieve the best possible outcomes.
Key Responsibilities:- Manage a portfolio of clients with physical or psychological injuries
- Organise appropriate intervention with various stakeholders
- Monitor progress of clients by maintaining regular contact
- Compile regular case records and write return-to-work plans
- Assist individuals to understand and resolve barriers by providing information and attend medical appointments
- Complete reports after assessments and meetings and communicate with medical professionals regarding diagnosis and treatment
- Current registration with ASSW is essential
- Minimum one year's experience in a similar role
- Bachelor's degree in Social Work
- Car and valid Driver's license
- Police check or willingness to obtain a police check prior to commencement
- Ability to build strong stakeholder relationships
