Community Care Specialist

Flinders and Upper North Local Health Network is seeking a skilled Community Care Worker to join our team. As a Community Care Worker, you will provide support to individuals who are aged, frail, or disabled, enabling them to maintain their independence and live in their own homes or residential aged care facilities.
Key Responsibilities:
- Provide personal care, cleaning, meal preparation, transport, and social support to clients
- Work collaboratively with clients, their families, and other health professionals to achieve quality outcomes
- Support clients to maintain their independence and participate in goal-setting and decision-making
- Contribute to a consumer-directed care model, where clients have control over their services and care arrangements
Requirements:
- Flexibility, conscientiousness, and empathy
- Ability to work autonomously in the community
- Strong communication and interpersonal skills
- Ability to maintain confidentiality and handle sensitive information
