About the Role

This Disability Support Worker position requires assisting participants of the NDIS to achieve their goals. It's a hands-on capacity building role that ensures a holistic approach to bettering people's lives. In this role, you'll provide evidence-based and recovery-oriented support to individuals discharged from correctional facilities, working collaboratively with the service team to transition them out of corrections and reintegrate them back into the community.

You'll assist clients to achieve their identified goals and aspirations, work closely with Community Mental Health, and provide support with day-to-day tasks and activities to develop professional working relationships with stakeholders and achieve the best possible outcomes for clients.

As a Community Rehabilitation Support Worker, you'll work with people with disabilities to achieve individual goals, dreams, and aspirations, and enhance their participation and inclusion in the community. This occurs through planned, person-centred activities and the development and enhancement of independent living and social skills.

Duties and Responsibilities

  • Helping participants in their transition out of corrections and reintegrating them back into the community
  • Assisting participants in recovering and learning essential life skills
  • Re-engaging in positive and supportive community-based services and resources
  • Assisting participants in making informed decisions to sustain current supports
  • Encouraging participants to grow as individuals and achieve their desired goals
  • Working with participants to identify goals and set action plans to achieve them
  • Improving and maintaining independent daily living skills
  • Preventing relapse and managing stress by building capacity for self-advocacy
  • Connecting to positive and supportive social and family connections

Required Skills and Qualifications

  • Degree/Diploma or Certificate in a human service-related field or demonstrated previous experience in correctional support services
  • Experience with Complex Mental Health Support
  • Proven ability to build individuals' capacity to learn new skills
  • WWCC & NDISWC including a police check or willingness to obtain
  • Full drivers license
