About the Role

As a Community Rehabilitation Support Worker, you will play a pivotal role in empowering individuals living with mental illness to achieve recovery and independence.

Key Responsibilities

- Foster trusting relationships with consumers
- Provide intensive case management support to address complex needs
- Facilitate referrals to relevant services and collaborations with community partners
- Conduct secondary consultations and data collection to inform effective care plans
- Work collaboratively with clinical case managers and other stakeholders

What We Are Seeking

- Proven experience working with people with psychiatric disabilities and complex presentations
- Ability to establish professional boundaries while building strong consumer relationships
- Excellent communication skills and knowledge of evidence-based recovery models
- Strong computer literacy and proficiency in using software applications

Essential Requirements

- Current Working with Children Check and Australian Working Rights
- Police check with disclosable outcomes considered
- Drivers licence and personal vehicle available for business use (if required)
