About the Role

We are seeking a highly skilled and experienced Dementia Support Specialist to join our team at HammondCare. As a key member of our team, you will play a critical role in delivering Australian Government funded dementia behaviour support services.

This is an exciting opportunity for someone with expertise in supporting people living with dementia to make a real difference in the lives of individuals and their families. You will have the chance to combine your caring, creativity, and allied health and/or clinical skills to solve complex problems and drive change.

As a Dementia Support Specialist, you will work collaboratively with families, caregivers, and clinical staff to understand how dementia impacts each individual. You will equip carers and clinicians with the knowledge and skills they need to prevent and respond to behaviour changes through modelling, coaching, and development of resources including behaviour support planning.

You will synthesise information from observation and written documentation to provide recommendations that are relationship-centred and informed by evidence-based best practice. Your reports will be contextualised to individual situations and people, effecting change on the ground and improving quality of life for people living with dementia.

In this role, you will undertake a range of assessments to help understand the holistic experience of the person and their care network. You will work in partnership with the broader HammondCare team to build capability across the home and aged care sectors.

Contribute to research, projects, and programs, and work innovatively to meet the changing needs of the sector. You will also work on other projects and assignments as required and in line with areas of skill, expertise, and knowledge.

What Do We Need From You?

  • Expertise in supporting people living with dementia
  • Relevant qualifications in Allied Health, Nursing, Aged Care or Mental Health essential
  • Experience in Australian residential aged care and/or community aged care
  • Be relatable and able to share knowledge in a way that supports change at all levels
  • High level of negotiation, communication, and relationship-building skills
  • Excellent problem-solving and decision-making skills
  • Ability to prioritise multiple tasks and meet deadlines
  • Excellent written and verbal communication skills
  • Excellent stakeholder management
  • A current Australian drivers licence and willingness to travel
  • Empathy and patience

Salary: $85,000 - $105,000 per annum (dependent on experience)
