Job Description:

We are seeking a highly skilled and compassionate Care Coordinator Specialist to join our team. As a key member of our care team, you will be responsible for coordinating the delivery of high-quality care services to our clients.

You will work closely with clients, their families, and other health professionals to ensure that each client receives tailored care that meets their unique needs and goals.

Your main responsibilities will include care planning, coordination, and monitoring of our clients' care packages to ensure that they receive the best possible outcomes.

You will also be responsible for building strong relationships with our clients, their families, and other healthcare professionals to ensure seamless communication and collaboration.

This is a full-time position based in the Brisbane CBD, and you will have access to company cars for in-home client visits.

Required Skills and Qualifications:
  • A minimum of 1 year of experience as a Home Care Package Coordinator
  • A relevant care-related qualification, such as a Diploma of Community Services or Case Management skill set
  • Solid understanding of the Home Care Package program, including care planning, budgets, and compliance requirements
  • Excellent communication, problem-solving, and organisational skills
  • Demonstrated ability to prioritise and multi-task
  • Good computer skills, including care databases, Outlook, MS Word, and Excel
