Job Title:
">Care Coordinator SpecialistOur organization seeks a dedicated Care Coordinator Specialist to provide support to individuals over 65 who have Home Care Package funding.
The ideal candidate will work within a close-knit and dynamic team to coordinate the delivery of high-quality care to clients, meeting and exceeding their expectations while also meeting business requirements.
Responsibilities include onboarding new Home Care Package clients, conducting regular care plan reviews in accordance with clients' changing needs, and scheduling services for new home care package clients in collaboration with the Scheduling Officer.
Additionally, the successful candidate will be responsible for monitoring clients' budgets and ensuring that they are able to lead fulfilling, independent lives through value-focused relationships and quality support services.

Requirements:
">To succeed in this role, candidates must possess qualifications in Community Services, Nursing, or an equivalent field.They should also demonstrate an understanding of the Aged Care Industry and experience working with Home Care Packages.
Strong interpersonal and communication skills, as well as the ability to effectively work and communicate with multidisciplinary teams, are essential.
Furthermore, candidates should be able to manage complex relationships and empathize with people from diverse backgrounds.

">This full-time position offers a competitive salary of $65,000-$75,000 per year.Employees will enjoy an 8-hour shift schedule, with opportunities for professional growth and development.
