About the Opportunity
Are you passionate about delivering high-quality care to older individuals? This is an exciting opportunity to join a dedicated team as a Case Manager and make a real difference in people's lives.
The role involves working closely with the Aged Care Team to develop and review care plans that enhance consumers' quality of life. To be successful, you will need to possess relevant qualifications such as Bachelor of Nursing, Allied Health or Social Work, and have experience in planning, coordination and delivery of services to older people living at home.
You will also need to demonstrate knowledge of Aged Care reforms, legislation, compliance requirements and quality standards. Additionally, you will be expected to work respectfully with Aboriginal and Torres Strait Islander peoples and their families.

Key Responsibilities:
- Develop and review care plans that meet the needs of consumers.
- Work collaboratively with the Aged Care Team to deliver high-quality services.
- Coordinate and plan services for older individuals living at home.
- Maintain up-to-date knowledge of Aged Care reforms, legislation and quality standards.
- Provide respectful and culturally sensitive care to Aboriginal and Torres Strait Islander peoples.
