We are seeking a passionate professional to join our team as a Health Care Connections Worker.

This role involves providing high quality, assertive outreach support to individuals experiencing homelessness.

The main aim of this position is to connect people in need with relevant local primary health care providers.

  • Empower clients to address barriers preventing them from accessing health services.
  • Build and maintain relationships with local primary health care providers and other relevant services.
About the Role

In this challenging yet rewarding role, you will:

  • Conduct comprehensive needs assessments with clients to understand their primary health care needs and current barriers to accessing services.
  • Develop clear and robust case plans with clients to support them in overcoming barriers to accessing primary health care services.
  • Refer clients to other services as required to manage any barriers they face.
  • Provide assertive, flexible, and regular outreach to target cohorts across three local government areas.
  • Proactively support clients in addressing barriers to accessing health care.
  • Help clients access the primary health care services they need.
