Job Title: Service Manager

We are seeking a highly skilled and experienced Service Manager to lead our operational teams and ensure that our services reflect the needs of our customers and deliver agreed quality outcomes.

Key Responsibilities:
  • Lead operational teams to ensure services meet customer needs and deliver quality outcomes.
  • Handle and resolve client queries, develop strategies for service improvement, and lead and educate client services staff.
  • Maintain business relationships with stakeholders, manage staff and data/records, and ensure the smooth operation of accommodations and other service sectors.
  • Ensure the health and well-being of staff and participants are addressed.
Main Tasks Include:Client Intake and Assessment & Support:
  • Provide rostered hands-on supports with participants.
  • Facilitate the intake and assessment of new clients, liaise with families, NDIS representatives, and allied health professionals to process referrals.
  • Negotiate funding and service arrangements for transitional support.
  • Develop support plans for each individual, ensuring adequate information is gathered regarding their immediate support requirements.
  • Develop documentation procedures for staff and review processes to ensure ongoing monitoring and evaluation of outcomes.
Staff Management:
  • Ensure the coordination, guidance, support, and performance management of individual staff members, students, and volunteers.
  • Adequate staff rostering as per client needs.
  • Commit to and lead positive behaviour support strategies and practices.
  • Ensure staff performance is in line with organisational objectives and within the parameters of their position descriptions and relevant awards.
Data and Records Management:
  • Comply with legislative and administrative requirements in the creation, handling, and reporting of information regarding individuals, staff, volunteers, and service delivery.
  • Adhere to Cocoon SDA Care policies relating to confidentiality and records management.
  • Ensure information and data collected is accurate and up-to-date.
About You:

We are seeking someone with:

  • STA, MTA, and SIL experience.
  • Service management experience.
  • Minimum 3 years NDIS service delivery experience.
  • In-home and community participation coordination experience.
  • Excellent rostering and time management skills.
  • Ability to go above and beyond with quality services.
  • Essential stakeholder relationships and communication skills.
  • Experience in ROC, Restrictive Practice, and high medical needs.
  • Experience in NDIS compliance.
