As a Supported Living Facilitator, you will make a tangible difference in the lives of people with disabilities.

Working collaboratively with individuals, their families, and support networks, you will help achieve their aspirations and enhance their overall well-being.

This critical role involves providing essential support in everyday living situations, focusing on personal autonomy, choice, and quality of life.

You will be responsible for:

  • Developing and implementing personalized plans that align with individuals' hopes and aspirations;
  • Enabling individuals to attain personal autonomy over their own lives and services;
  • Providing emotional support and guidance to help individuals navigate life's challenges.

To excel in this position, you will need to possess excellent communication skills, both verbal and written, as well as the ability to work effectively within a team-oriented environment. Flexibility and reliability are also essential qualities for this role.

A strong understanding of the Disability, Aged, and Community sector is desirable but not mandatory. You will require current NDIS Worker Screening check and Blue Card certifications.
