Job Summary

A Supported Living Facilitator plays a vital role in supporting individuals with disabilities to live independently.

Main Responsibilities:
  • Collaborate closely with clients, their families and support networks to achieve life goals.
  • Provide personal care and support in everyday living situations, prioritizing choice and enhancing quality of life.
  • Develop and implement individual plans that focus on the client's hopes and aspirations.
  • Enable clients to make informed decisions about their lives and services.
  • Maintain confidentiality and respect the rights of clients and their families.
Essential Skills and Qualifications:
  • Effective communication and teamwork skills are essential.
  • Flexibility and reliability in meeting client needs is crucial.
  • The ability to communicate in spoken and written English is required.
  • Experience in the disability sector is desirable but not necessary.
  • A current NDIS Worker Screening check and Blue Card are mandatory.
  • An Open Driver's License and current CPR/First Aid Certificate are also required.
