Job Overview

We are seeking a dedicated and compassionate professional to join our team as a Client Outcome Facilitator.

The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work collaboratively with clients and stakeholders to achieve positive outcomes.

  • Key Responsibilities:
  • Providing holistic support throughout all facets of the client journey
  • Case management that focuses on client needs and fosters client capacity toward independence
  • Assisting clients to access community resources and services
  • Delivering advocacy on behalf of clients and ensuring confidentiality of client information at all times

Requirements:

  • Minimum Certificate III in Community Services or related field
  • Knowledge of and ability to work within relevant legislation and guidelines relating to the delivery of case management and safety and risk within the program area of operation
  • Previous experience in the homelessness sector is preferred or equivalent within the program area of operation
  • Personal aptitude for working within set guidelines and adhering to all relevant policies and procedures
  • Well-developed interpersonal, communication, and organisation skills including the ability to exercise judgement and contribute to a team environment
  • Current Queensland Driver's License required
