Community Engagement Specialist

We are seeking an experienced Community Engagement Specialist to bridge the gap between our organisation and the community, identifying service gaps and developing strong relationships with stakeholders.
This is a client-centered role that requires a flexible, responsive, creative, and collaborative approach. As a key member of our team, you will be responsible for:
- Proactively engaging with local communities to identify service gaps and resources available to our clients;
- Building and maintaining relationships with community organisations, businesses, and service providers to enhance our referral network;
- Coordinating the onboarding process of new Home Care Package clients;
- Organising support to clients, monitoring, and reporting on their progress;
- Assisting clients in identifying ongoing needs and goals to support them to remain in their own home.
To succeed in this role, you will require:
- Experience in community engagement, networking, marketing, outreach, sales, or similar roles, preferably within the aged care sector;
- Knowledge of Case Management principles, Home Care Package Guidelines, and Funding body;
- A strong client focus based on their goals, preferences, and needs;
- The ability to work within a team as well as autonomously;
- Excellent organisational skills and ability to manage multiple priorities;
- Excellent verbal and written communication skills and strong computer skills and experience with client database systems;
- A current National Police Check; and
- A full Driver's License.
