The Clinical Care Coordinator role is an exciting opportunity for a registered nurse to take on a leadership position in aged care.
Gilgandra Shire Council seeks a dedicated professional to provide clinical support and guidance to their Aged Care & Disabilities team.
About the PositionThis full-time role involves participation in an on-call roster for after-hours support. The successful candidate will lead and support the service with a focus on delivering high-quality clinical care.
Key Responsibilities- Collaborate with residents, allied health professionals, and stakeholders;
- Develop and manage comprehensive care plans, including wound care and outcomes within the facility;
- Supervise staff clinical practice, ensuring adherence to best practices and policies;
- Promote teamwork and coordination among care teams and across service types;
- Ensure all documentation meets policy, procedure, ACFI, and accreditation standards;
To be considered, applicants must be a Registered Nurse with current registration with AHPRA and relevant aged care experience.
Strong interpersonal skills, effective communication abilities, and computer literacy, specifically Microsoft Office Suite, are essential.
