About the Role

This role oversees and enhances the delivery of Aged Care services. Duties involve processing referrals, aiding clients in commencing services and adjusting care plans to suit evolving needs.

Coordinate home visits to ensure client needs are met and work in partnership with them and their families.

Key Responsibilities
- Conduct comprehensive client assessments, develop profiles and care plans
- Monitor and review care plans regularly
- Collaborate effectively with internal stakeholders to meet client needs
- Develop strategies for identified needs of Aboriginal and Torres Strait Islander clients
- Represent at external forums and meetings

Skills & Experience
- Demonstrated experience in Aged Care or clinical case management
- Understanding of Aged Care funding models
- High competency in technology utilisation
- Ability to identify and implement risk mitigation strategies
- Strong communication, influencing, liaison and negotiation skills
- Established problem solving and conflict management skills
- Ability to multi-task and work autonomously
