Job Title: Geriatric Care Specialist
Role Overview:
Our team provides care for older patients with complex needs, including acute medical care and management of geriatric syndromes.
The Geriatric Care Specialist will work effectively with a multidisciplinary team to ensure patients receive the right care at the right time. Key Responsibilities include providing leave cover, admitting and discharging patients, working collaboratively with team members, facilitating communication with other staff and healthcare providers, documenting patient information, communicating with patients and their families, participating in quality activities and training requirements, and contributing to engagement with stakeholders.
Required Skills and Qualifications:
- Medical degree or equivalent qualification
- Postgraduate qualifications in geriatrics or related field
- Registration as a medical practitioner
- Experience in acute medical care and geriatric medicine
